Listed below are permission notes and forms that may need to be completed and returned to school.
Please note that many forms are available online through School Bytes - Parent portal.

Application for Extended leave - Travel (interstate or overseas)
Bring your own device agreement (BYOD)
Exchange of information (must be completed for school to discuss information)
Parent/carer request for Allied Health Professional (Therapists - please complete for each therapist)
Note to parent/carer all Allied Health Professionals MUST complete agreements and forms before working with students
Permission to publish (photography)
Health Support Plans
Students who have medication/health support at school, will have a Health Support Plan completed by school.
A copy of Dr letter will be required for medication (name of medication, times and whether medication can be crushed if required)
A copy of support plans are required from Allied Health Professionals for (e.g diabetes plan, hoisting/transfer from wheelchair, eating/feeding plans and suctioning plan)
